What the Job Involves
This role involves completing Care Act Assessments, Reviews, and managing safeguarding and care coordination under the CPA Framework. You will manage a caseload of 25 clients and participate in a Duty Rota, which may include screening, triaging, and managing new referrals. You will be an integral part of an integrated Mental Health and Wellness Team, working collaboratively to deliver comprehensive support to clients.
