You are a registered Social Worker with Social Work England, holding a qualifying degree, and have a deep understanding of current legislation. You are a dynamic, self-motivated, and professionally confident individual with a drive to potentially pursue advanced qualifications. Collaboration is key to your success, and you have a proven track record of building strong partnerships with external agencies. Additionally, you hold a full, clean driving license and are eager to engage with the community across Herefordshire's vibrant locations.
What the Job Involves
Your role will include conducting a wide variety of adult social work activities such as Care Act Assessments, person-centered support planning, Mental Capacity Assessments, and Section 42 Enquiries. You will work with diverse adult populations, including those with physical and learning disabilities and mental health needs, using a strength-based approach to empower and promote independence. Additionally, you will engage in Continuing Healthcare (CHC) and section 117 related tasks, collaborate with partner agencies, and contribute to a culture that supports ongoing professional growth and development.
