Who You Are

A committed and caring individual with the ability to work in a dynamic and fast-changing service. You are flexible, focused, and possess good observation and recording skills. You have the capability to train to achieve the relevant HSc qualifications required for the post. The ability to speak Welsh and other community languages is an advantage. You must have a full clean driving licence and access to a car. This position requires female-only applicants.

What the Job Involves

You will join the Community Resource Team (CRT) Homecare in Cardiff, supporting citizens to prevent unnecessary hospital admissions and facilitate discharges. This role involves working closely with health professionals to help service users achieve their reablement goals and live independently in their own homes. You will assess service users to build and implement a plan to support their wellbeing goals. The role includes shift work according to various rotas, including evening and weekend hours, with duties allocated via a mobile device.
